
CentraCare celebrated its one year anniversary of serving older adults in Calhoun County on April 1, 2010. The program is located at the Burnham Brook Community Center in Battle Creek, and its mission is to help Calhoun County residents 55 years of age and older remain in their homes, apartments, or other community setting of choice.
The program, also known as PACE (Program of All-Inclusive Care for the Elderly), is a nationally recognized model of care with four Centers in Michigan at this time. CentraCare provides care for older adults receiving Medicare and/or Medicaid benefits who can be maintained safely in their place of residence with support from program services.
All services are provided free of charge to Medicare and Medicaid beneficiaries. CentraCare provides team-based comprehensive medical management and coordinated care through the clinic and day program at our center. High quality transportation is provided by LifeCare Ambulance Service for participants to come to the day center. While at the CentraCare Day Center, the participant is able to receive meals, activities, rehabilitation therapy when needed, mental health services and physician/nursing care. Also, if necessary, CentraCare can provide transportation for specialty service appointments. Our staff include physicians, nurses, nursing assistants, social workers, a dietician, and pharmacist. We also have recreational, occupational, and physical therapists on staff.
Our intake specialist can help people apply for Medicaid when needed. The income levels to qualify for Medicaid for this program are higher than some of the other programs in the community, so even if someone has been denied Medicaid in the past, they may qualify for this program. Once someone is enrolled at CentraCare the program assumes financial responsibility for medications, medical supplies, diabetic supplies and other healthcare expenses.
